Elevated Health provides patients with modern medical care, but brings back the convenience and comfort of old time medicine when the provider came to you.

Our STORY

Elevated Health was born out of a desire to provide medical care to EVERYONE in our community despite their location or insurance status. This is why Elevated Health ia a CASH PAY ONLY medical practice.

The cost of every visit will be known up front to the patients. Additional cost, such as prescription medications, will be dependent on pharmacies and if the patient will be using their insurance or paying cash.

We will offer mobile urgent care, direct primary care (we come to you), telemedicine visits and mobile IV therapy during after hours to start, but will be transitioning to full time hours within the next few months.

Max Taylor, FNP-BC

Max is a Family Nurse Practitioner and Sevier County native. He grew up in Seymour and graduated from Seymour High School. From there, he proceeded to get his nursing degree from Walters State and finished his journey with his Masters in Nursing from the University of South Alabama. Max has served this community as a RN, paramedic and Nurse Practitioner. He is married to his wife, Shannon, and has two amazing kids, Sage and Mason.
